  1. Extracurricular Activities: Please list all extracurricular activities you participated in during high school and provide a brief overview of activity.

  1. Community Service: Please list all community service activities/projects you have been actively involved in during the past three years. Please specify dates and organizations associated with each activity.

  1. Awards and Recognition: Please list all awards received over the past five years, listing the name of the organization(s) giving the award

  1. The following documents must be attached to your application upon submission for consideration:

High School Transcript: Applicants must provide a sealed copy of their high school transcript bearing the seal of the Registrar’s office issuing the transcript.

College Acceptance Letter: Applicants must attach a copy of their acceptance/enrollment letter from the college, or program they plan to attend.

Documentation of Financial Need: Applicants must provide documentation of any/all financial assistance including FAFSA awards (grants and/or loans), awarded scholarships from other organizations (faith based,community, or service groups).

Letters of Recommendation: Applicants must submit two letters of recommendation from a teacher, guidance counselor or principal from their high school. The name, contact telephone number, and title of the person completing the letter must be included.

Typed essay on the following topic: What/who are the inspirations for your chosen college major, and how will you utilize your education to impact future generations?
